Rates
$130 per 60-minute initial session then $110 per 60-minute
for all following sessions.
Insurance
Services may be covered in full or in part by your health
insurance or employee benefit plan. It is your responsibility to check your
coverage carefully before scheduling an appointment by asking the following
questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- What is my co-pay or co-insurance?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
- Do I need an authorization?
- Is this provider in network? Provide them with one of these numbers: NPI 1275815631 or Tax ID 46-1570298. They may require my name and office address as well.
Reduced Fee
Reduced fee services are available on a limited basis.
Payment
Although cash or check is preferred, all major credit cards
are accepted as well as payment through Venmo, PayPal, Zelle, or Cashapp . Be mindful that a 1.75% to 3.5% service fee of the amount
charged will also be charged, if using a credit card or online options. Due to the rising costs of credit card
companies fees & bank fees, this cannot be avoided. All deductibles
and co-pays must be paid up front. You will not be billed.
Cancellation Policy
Insurance will not pay for missed or canceled appointments.
Your appointment time is reserved exclusively for you. In order to maintain this level of commitment to my clients, I require at least 48-hour notification, if you need to change or cancel your reserved appointment time.
I do my best to respect your time and ask you to respect my time and other clients’ needs by keeping your scheduled appointments. Each appointment time slot is important and cannot be recovered, if a client chooses not to keep their appointment. Please keep in mind that each skipped or missed appointment time is not just time lost but may also prevent me from providing care to other clients.

• It is your responsibility to provide me with a working telephone number to allow me to communicate important information. An email address may also be provided as well.
• I may or may not send a courtesy appointment reminder via email and/or text. Please keep in mind, these are COURTESY reminders, if I do, and IT IS SOLELY AND FULLY YOUR RESPONSIBILITY to remember your appointment times.
If you do not show up for your scheduled therapy appointment, and you
have not notified me at least 48 hours in advance, you will be required to
pay a $80 fee. I know some things are out of your control.
So, in some cases, the fee will be waived to $40. You will
have to pay all fees before another appointment will be scheduled. No exceptions.
Because my evening (after 4 pm) and weekend (Saturday) time spots are in high demand, if you cancel or miss two sessions in a row for an evening or weekend appointment, you will not be scheduled in those time slots in the future. No exceptions.
